To request an urgent appointment for the same day (Monday to Friday)
- use our appointment request form, Monday to Friday from 7:30am to 4pm. We will respond on the same day during opening times. Please make sure you are available to attend on the same day.
When you get in touch, we’ll ask what you need help with.
With the information provided we will choose the most suitable doctor, nurse or health professional to help you.
If you are unable to use the online form please call in or telephone reception and the receptionist will add the information for you.

Routine appointments
To request a routine / follow up appointment
- use our appointment request form, Monday to Friday from 7:30am to 6:30pm. We will respond within 24 hours during core opening times.
When you get in touch, we’ll ask what you need help with.
With the information provided we will choose the most suitable doctor, nurse or health professional to help you.
If you are unable to use the online form please call in or telephone reception and the receptionist will add the information for you.

Your appointment
However you choose to contact us we may offer you a consultation:
- by phone
- face to face at the surgery
- on a video call
- by text or email.
Appointments by phone, video call or by text or email can be more flexible and often means the doctor or nurse can help you sooner.
To cancel your appointment:
- use your NHS account (on the NHS website or in the NHS app)
- fill in an online form
- reply CANCEL to your appointment reminder text message
- phone us on 01255 222121, Monday to Friday, when we are open
Extended access appointments are available on Saturdays. Extended access appointments are rotated between all 3 surgeries in PCN: Mayflower, Great Bentley and St James.
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Please tell us:
- If there’s a specific doctor, nurse or other health professional you would prefer to respond.
- If you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email.
- If you need an interpreter.
- If you have any other access or communication needs.
Please submit an online request via askmygp or phone 01255 222121 as soon as possible (from 8am) to request a home visit.
Our clinicians will call and triage your request to understand your issue and assess whether a visit is clinically required. It may be that your problem can be dealt with by telephone advice. If necessary, they will arrange for the most appropriate member of the clinical team to visit.
Home visits are only available for patients who are housebound because of illness or disability. We ask that all pets are removed from the room the patient is in and that rooms are well ventilated if smoking has taken place.
